Preparing for Your Appointment

What to Bring

Please bring the following items with you for your first appointment:

• Your prescription or referral for therapy, if applicable.
• Your insurance card.
• Your driver's license or other photo ID.
• A list of all medications you are currently taking.
• Any questions you may have for us.

What to Wear

Your privacy, dignity, and comfort are of utmost importance to our staff. For ease of evaluation and treatment, we recommend that you wear or bring comfortable clothing suitable for activity. Your evaluation and treatment may require shorts or a gown, which will be provided by your therapist.

Before You Arrive

Please allow adequate time to complete necessary paperwork prior to the start of your initial visit. This can take 15-30 minutes, depending on your comfort level.

When traveling to and from our offices, please note that additional time may be necessary due to construction on Route 28.

Fox Chapel Physical Therapy offers free, convenient parking.

There are no stairs at our office.

O'Hara Center

Our O’Hara Center office is located at 1339 Freeport Road. From either direction on Freeport Road, turn into the O’Hara Center/Beer Nutz Plaza parking lot and continue up the hill to the right. Our office is located on the lower level of O’Hara Center, facing Freeport Road.
