Making An Appointment

Whether you've been recently referred by a physician and have a prescription for treatment or are seeking an evaluation from our practice through Direct Access, it will be helpful to gather the following items and have them in front of you before calling to schedule your first appointment:

Your prescription or referral for therapy, if applicable.
Your insurance card and information.
A list of days and times that work best for your scheduling needs.

Our front desk staff will ask you basic questions about why you have been referred to us, your insurance information, and your scheduling needs. Cancellations and Schedule Changes

If you need to cancel or reschedule an appointment for any reason, please contact our office as soon as possible. Every effort will be made to reschedule at a time most convenient for you.
